4. Emergency Waiver and Legal Consent

In case of an emergency, I give permission for the GRCC staff to perform life-saving procedures on all of my children registered with GRCC. Furthermore, I give the school permission to secure treatment by a medical professional for any advanced medical or surgical procedure that may be necessary for the welfare of my child(ren). I grant GRCC permission to transport my child(ren) in a medical emergency as needed.

I attest that all the information provided here is accurate and correct. I have the authority to make medical and legal decision on behalf of the child(ren) registered. I agree with the above medical treatment agreement policy.

In addition, in consideration of being allowed to participate in any way in the Glen Rose Christian Co-op, and related events and activities, the undersigned:

Assumes all the foregoing risk and accepts personal responsibility for damages following any injury, permanent disability, or death.
Releases, waives, discharges, and covenants not to sue Glen Rose Christian Co-op, its affiliated clubs, their respective administrators, director, teachers, volunteers, board members, and other employees of the organization, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, Grace Community Church, the owners, and lessor of premises used to conduct the event, all of which are hereinafter referred to as “release” from any and all claims, demands, losses, or damages on account of injury, including death or damage to property, caused or alleged to be caused in whole or in part by negligence of the releases or otherwise.
